Best evidence application of phase Ⅰ cardiac rehabilitation in transcatheter aortic valve replacement patients

Objective To investigate the application of best evidence for phase I cardiac rehabilitation in transcatheter aortic valve replacement(TAVR) patients into clinical practice and to evaluate its effectiveness. Methods The best evidence for phase I cardiac rehabilitation in patients with TAVR was summarized using an evidence-based care approach,and an evidence-based practice protocol was developed. The best evidence application strategy was constructed after a baseline review and analysis of barriers,and it was implemented in the cardiology department of a tertiary care hospital in Zhejiang Province from September 30 to December 31,2020. The indicators of patients before and after the application of evidence and the implementation rate of the reviewed entries were compared. Results The length of stay of patients after TAVR was shortened from 5(1,7) d at the baseline review to 3(1,6) d(P=0.016). The percentage of cardiac function of New York Heart Association(NYHA) class I at 30 days postoperatively increased from 31.63% to 72.09%(P<0.001),and the Katz index also improved(P=0.032). After the application of best evidence,the rates of review indicators increased from 0~77.55% to 59.30%~100%,with a statistically significant difference(P<0.001). Conclusion Best evidence application for phase I cardiac rehabilitation in patients undergoing TAVR can shorten patient length of stay,improve patient clinical outcomes,and standardize nurse behaviors.

Analysis on the status and influencing factors of diet self-efficacy of the elderly in nursing homes

Objective To investigate the current status and influencing factors of diet self-efficacy among the elderly in nursing homes. Methods We used the convenience sampling method to select 350 older adults in 3 nursing homes in Yinchuan from August 2020 to December, using a General Information Questionnaire,Nutrition Knowledge and Attitude Questionnaire,Micro Nutrition Assessment Scale,and Diet Self-Efficacy Scale to conduct a questionnaire survey. The random forest model combined with multiple linear regression was used to analyze the influencing factors of diet self-efficacy. Results The total score of diet self-efficacy of the elderly in nursing homes was(54.98±8.04) points,and the scoring rate was 68.6%,with an average score of(3.43±0.50) points for all items. The random forest sorting showed that the top 5 factors affecting diet self-efficacy were nutrition attitude, exercise time, nutrition knowledge,nutrition status,and highest education. The multiple linear regression analysis results showed that nutrition knowledge, nutrition attitude,nutrition status,exercise time,education,and previous occupation were the influencing factors of diet self-efficacy,which were similar to the random forest results. Conclusion The diet self-efficacy of the elderly in nursing homes is moderate. Nutrition attitudes,knowledge,nutrition status, exercise time,and highest education are essential factors affecting diet self-efficacy. Nurses should comprehensively consider the above factors to develop elderly nursing plans to promote their diet self-management confidence and ability, thereby ameliorating their nutritional status.

Congenital tufting enteropathy is rare to see in clinic,and it could be life-threatening due to intractable diarrhea in the early stage. The nutrition management was the most important and difficult part of nursing care. This study aimed to summarize the nutritional support experience of a case with congenital tufting enteropathy. In this case,we provide the personalized nutrition support based on nutrition screening and evaluation,and carry out parenteral nutrition support as soon as possible to help the child get fully potential of growth,as well as to get the metabolism balance. Besides,the timely enteral nutrition support was also used to improve the nutritional status of the child. Prevention and timely intervention for common complications were used to achieve clinical benefits. After a 60-day nursing care,the child’s condition was well-controlled with steady increasing in height and weight.

Objective To explore the mediating effect of professional self-concept between job satisfaction and turnover intention among nurses with master’s degree. Methods Totally 408 nurses with master’s degree from 23 provinces were recruited by convenience sampling method. They were investigated by the general information questionnaire,the Job Satisfaction Scale(JSS),the Nurses’ Self-Concept Questionnaire(NSCQ),and the Turnover Intention Questionnaire(TIQ). Results The results of path analysis showed that job satisfaction and professional self-concept could negatively predict turnover intention directly,and professional self-concept played a mediating role between job satisfaction and turnover intention. Conclusion It is suggested that nursing managers should adopt various measures to cultivate the positive professional self-concept of nurses with master’s degree and improve their job satisfaction in order to reduce their turnover intention for the construction of nursing teams with sustainable development.

Objective To explore and apply the nursing prevention and control measurement for patients with obstruction of nasointestinal tube after radical gastrectomy for gastric cancer. Methods Through literature review and Delphi expert consultation,the nursing prevention and control measurement was designed for patients with obstruction of nasointestinal tube after radical gastrectomy for gastric cancer. 2 wards of tumor surgery in a Leval A tertiary hospital in Henan Province from October 2020 to February 2021 were conveniently selected,with Ward A including 57 gastric cancer patients as the intervention group and Ward B including 55 gastric cancer patients as the control group. The intervention group received the measurement based on the conventional nasointestinal tube care of the control group. The rate of tube obstruction between Ward A and B,the average time in handling the tube obstruction,and the nurses’ cognition and behavior scores in the intervention before and after the intervention,the satisfaction of the nurses with the measurement in the intervention group were compared. Results The rate of tube obstruction in the intervention group was significantly lower than that in the control group (P<0.05). The average time spent by nurses in the intervention group to deal with the tube obstruction was significantly less than that in the control group(P<0.05). The cognition and behavior scores of nurses in the intervention group for the prevention and control of nasointestinal tube obstruction were higher than those before the intervention,and the difference was statistically significant(P<0.05). The satisfaction score of nurses in the intervention group on the record was (36.16±1.92). Conclusion The application of the nursing prevention and control measurement for patients with obstruction of nasointestinal tube after radical gastrectomy for gastric cancer can effectively reduce the rate of tube obstruction in patients with nasointestinal tube,reduce the average time spent in dealing with the tube obstruction,and improve nurses’ cognition and behavior of nasointestinal tube obstruction prevention and control.

Construction of early motor rehabilitation nursing program based on Modified Barthel Index grading for stroke patients

Objective An early motor rehabilitation nursing program for stroke patients based on Modified Barthel Index(MBI) grading was constructed to guide clinical nurses to perform precise early motor rehabilitation for stroke patients,in order to provide a basis for developing guidelines for rehabilitation nursing care of stroke patients.Methods Based on literature review and expert meetings,a draft of motor rehabilitation nursing program for stroke patients was constructed. The Delphi method was used to conduct 2 rounds of expert consultations to determine the early rehabilitation nursing program for stroke patients.Results In the first round,17 experts were consulted,and the response rate was 94.12%;in the second round,16 experts were consulted,and the response rate was 100%. The expert authority coefficient was 0.90. After 2 rounds,the coefficient of variation of items was 0 to 0.24,Kendall’s coefficient was 0.226(P<0.001) and the importance value was 4.13 to 5.00. Finally,an early motor rehabilitation nursing program classified by MBI was formed,including 5 first-level indicators(fully dependent,heavily dependent,moderately dependent,mildly dependent and basic self-care) and 47 second-level indicators(rehabilitation).Conclusion The nursing program constructed in this study is reliable and scientific,and it can provide references for the research and practice of rehabilitation training for stroke patients.

Objective This paper summarizes the experience of establishing nursing emergency personnel reserve bank based on information network technology in large general hospitals during the epidemic of coronavirus disease 2019(COVID-19). Methods It Includes adjustment of organizational structure system,establishment and management of nursing emergency personnel reserve bank. The personnel reserve bank was constructed based on the formation principles and procedures;the maintenance of emergency personnel’s basic information,the training of COVID-19 knowledge and related skills,and the multi-dimensional psychological support and humanistic care were implemented based on the hospital information platform. Results As of February 15,2020,a total of 553 nurses had been admitted to the reserve bank,of whom 260 were on standby and 293 were involved in the frontline epidemic prevention and control. Conclusion Nursing emergency personnel reserve bank based on information network technology in our hospital ensured efficient manpower dispatch and dynamic personnel management during the epidemic,which accumulated experience for the follow-up disaster aid.
